NCCN CEG Guidelines (V1.2025): EPCAM Post

The National Comprehensive Cancer Network (NCCN) released updated Genetic/Familial High-Risk Assessment: Colorectal, Endometrial, Gastric Cancer guidelines on June 13th, 2025 (Version 1.2025). Included in these new guidelines are updates to the surgical options for Lynch carriers and updates to the EPCAM content as outlined below:

LS-C 1 of 5 (Page 50)

  • Sarcoma row added with footnote m: In the Prospective Lynch Syndrome Database, a total of 14 sarcomas (10 osteosarcomas and 4 soft tissue sarcomas) were identified, primarily in individuals with MSH2 PV (Reference: Dominguez-Valentin M, et al. Int J Cancer 2021;148:512-513).

LS-C 5 of 5 (Page 54)

  • Management for sarcoma: There is no clear evidence to support surveillance for sarcoma in Lynch Syndrome (LS).
