The National Comprehensive Cancer Network (NCCN) released updated Genetic Familial High-Risk Assessment Colorectal, Endometrial, and Gastric Cancer guidelines on October 3rd, 2024.
Updates include ⤸
Added the following to testing being considered: Personal history of colorectal or endometrial cancer at or older than age 50, and:
- Tumor not testing for mismatch repair deficiency (e.g., MSI status)
- Tumor tested and found to be mismatch repair deficient
Revised information about EPCAM gene (which has usually been lumped together with MSH2):
- Risk of gynecologic cancers is variable and related to the extent and location of the deletion of EPCAM and proximity to the MSH2 promoter.
- Counseling and surveillance recommended based on family history and shared decision making, and later age of risk-reducing surgery (similar to PMS2-related guidelines) may be appropriate.
